Park City Academy Spirit Wear

 

General Information

Spirit Wear includes both clothing and accessories. Most items include the new Park City Academy logo and are approved as school uniforms. Accessories may be monogrammed at extra charge.

A portion of the proceeds from all Spirit Wear items goes towards supporting our PFA.

Ordering Information

An online order form is provided for your convenience. NOTE: THE COMPUTER YOU ARE USING MUST BE CONNECTED TO A PRINTER! Before you open the form, please read through these simple instructions:

1. Click HERE to open the form (it will open in a new window, so you can see both the form and these instructions).
2. Choose items by selecting a size from the dropdown Size Field, then click in the corresponding Qty field and enter a quantity.
3. Choose additional items, as desired - you can also go back and modify any previous selections. Note that the form automatically calculates the cost.
4. Fill in your name and telephone number (required items) - email address is optional
5. Print the completed form and either bring it into Park City Academy or fax it to Acme Threadware at 435.487.9050. Acme will contact you for payment.
